Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to complete your new patient paperwork online. There’s no need to print anything; just fill out and submit the forms from your phone, tablet, or computer. This will allow us to attend to your medical needs more promptly than if you were to complete them upon arrival for your appointment. This helps us stay on schedule and respect everyone’s time.
Financing
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. We also participate in Cherry and Care Credit payment plans. We can tailor a treatment plan to align with your schedule and budget.
Insurance
While we work with all insurance plans, coverage and reimbursement for periodontal care will work a little differently. Also, certain procedures may not be covered by insurance. It’s important to contact us early so we can discuss your insurance coverage and the best payment option for your procedure:
